Position Summary:
Utilizes an interdisciplinary approach to organize, coordinate, monitor, evaluate and modify plans of care, focusing on complex medical and psychosocial needs of the Hospital's patients and their families. Performs pre-admission screening review of patients. Assesses and plans interdisciplinary plan of care that effectively manages patient along the continuum. Ensures appropriate use of resources and coordinates discharge planning.
Qualifications:
EDUCATIONAL REQUIREMENTS:RN, licensed in MassachusettsBSN or commensurate experienceCase Management Certification preferredComputer literacy in Word, Excel, Outlook
EXPERIENCE REQUIRED:5 years clinical experience RequiredMinimum 1 year Case Management Experience RequiredCase Management Certification preferredWorking Knowledge of InterQual Criteria preferredOutcomes/resources management skills Midas and MediTech Experience preferred
PROFESSIONAL LICENSURE AND CERTIFICATION REQUIRED:RN, licensed in Massachusetts
BSN or commensurate experience
Case Management Certification preferred
Computer literacy in Word, Excel, Outlook
INTERPERSONAL RELATIONSHIPS:Communicates effectively with patients, families, visitors, and all members of the health care team.
PHYSICAL REQUIREMENTS:Physical health sufficient to meet the ergonomic standards and demands of the position and its location.
